To fix this issue, a manual step is required.
To upgrade from 4.x to 5.x, you will need to delete the "editPanel" and recreate it after opening 4.x project in 5.x Studio. After recreating editPanel, then set editPanel's liveForm property to whatever the LiveForm in the project. Attached is the modified working project.
